DISCLAIMER & TERMS OF SERVICE:
By scheduling any service offered, communicating with Alexis Mendez, and/or paying for any services Alexis Mendez offers, you are stating that you have read this legal disclaimer and Alexis Mendez’s terms of service ("Agreement"), that you understand, voluntarily and willingly enter into, agree with, and accept the following:
1. You are 18 years of age or older.
2. 100% accuracy cannot be guaranteed. By its very nature, working with Spirit and with psychic energy requires the participation of at least one human and humans are imperfect creatures. You acknowledge and understand that, while Alexis Mendez is knowledgeable and has expertise in this area of healing, she is not working as a licensed therapist in this capacity and these sessions do not count as therapy services.
3. Any information you receive from any and all communication or consultations with Alexis Mendez is for educational, spiritual, and entertainment purposes only and is not intended to, nor should it ever, take the place of any medical, legal, financial, traditional psychological or other professional advice. Alexis Mendez shall not be responsible for any decisions made or actions taken by you or anyone based upon consultations or communications with her. The choices you make and the actions you take are solely your responsibility. You agree to completely hold blameless, harmless and absolutely indemnify, Alexis Mendez her business and practice from any and all liabilities, claims and expenses for any loss or damage resulting from your use of the information and services provided to you by Alexis Mendez as well as any breach by you of any covenant, term and/or condition contained in this Agreement.
4. Fees for services must be received on the day of service, cash, checks made payable to cash (at your own risk if mailing), Venmo, Zelle, or Square are the only accepted methods of payment.
Sessions/Reads can be done either in-person, through FaceTime, Zoom, or over the phone. If you are scheduled for an in-person session, you may bring your payment to your appointment. Give 24 hours’ notice if you should need to cancel your scheduled service or should Alexis Mendez need to cancel your scheduled service, you will be offered the option of rescheduling that service. If your scheduled service is not rescheduled any payment received will be promptly refunded.
5. Any and all correspondence between yourself and Alexis Mendez is intended for you only. Copyright laws and confidentiality agreements apply.
6. The client may use a recording device during communications, reads, and/or sessions for the purposes of record keeping and to assist in the process of the services she provides. These recordings and all other personal information is kept strictly confidential at all times. You fully understand and agree to this.
7. You have had the opportunity to ask questions and you fully understand and of your own free will choose to enter into this legally binding disclaimer and all stated terms of service.
8. YOU HEREBY ACKNOWLEDGE, AGREE AND ACCEPT THAT ANY AND ALL SERVICES PERFORMED OR PROVIDED TO YOU BY ALEXIS MENDEZ ARE DONE SO WITHOUT ANY REPRESENTATIONS OR WARRANTIES OF ANY KIND, EITHER EXPRESSED OR IMPLIED, INCLUDING, BUT NOT LIMITED TO ANY IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE AND NONINFRINGEMENT. ALEXIS MENDEZ DOES NOT WARRANT OR MAKE ANY REPRESENTATIONS REGARDING THE USE OF HER SERVICES OR THE RESULTS OF THE USE OF HER SERVICES OR THE CORRECTNESS, ACCURACY, RELIABILITY OR OTHERWISE OF HER SERVICES. YOU UNDERSTAND, ACKNOWLEDGE AND AGREE THAT YOU SHALL AT ALL TIMES ASSUME TOTAL RESPONSIBILITY AND RISK FOR USE AND/OR RELIANCE UPON ANY SERVICES AND INFORMATION PROVIDED TO YOU BY ALEXIS MENDEZ AND THAT ALEXIS MENDEZ SHALL NOT BE RESPONSIBLE NOR LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL, EXEMPLARY, PUNITIVE, OR OTHER DAMAGES ARISING OUT OF OR IN ANY WAY RELATING TO ANY SERVICES AND/OR INFORMATION PROVIDED TO YOU AND/OR YOUR RELIANCE UPON SAME. UNDER NO CIRCUMSTANCES SHALL ALEXIS MENDEZ BE RESPONSIBLE FOR ANY LOSS OR DAMAGE (INCLUDING PERSONAL INJURY OR DEATH) RESULTING FROM YOUR USE OF HER SERVICES.
